Applicants are required to submit the following documents:

1. Duly accomplished Nurses’ Application Form (NAF)
2. NSO issued Certificate of Live Birth
3. Transcript of Record with scanned picture and remarks: “For Board Examinations Purposes”
4. Record of OR-DR Cases (for 1st takers only)
5. Summary of Related Learning Experience – (for 1st takers only)
6. Four (4) pieces passport size (1 ½ x 1 ½) colored picture with complete name tag in white background
7. Community Tax Certificate (cedula)
8. NSO issued Certificate of Marriage (for married women)
9. One (1) window mailing envelope
10. Valid Identification Card
11. Examination Fee of P900.00

There will be NO EXTENSION of the deadline. All applicants are advised to file early to avoid overcrowding and long lines. Applicants with complete and appropriate documents will be accommodated.

Successful examinees are required to register personally and to sign in the Roster of Registered Professionals. Registrant are to bring the following: duly accomplished Oath Form or Panunumpa ng Propesyonal, current Community Tax Certificate (Cedula), 2 pieces passport size picture (colored with white background and complete name tag), 1 piece 1” x 1” picture (colored with white background and complete name tag), 2 sets of metered documentary stamps, and 1 short brown envelope with name and profession; and payment of the Initial Registration Fee of P600 and Annual Registration Fee of P450 for 2010-2013.

The oathtaking ceremony of the newly passed nurses and those who have yet to take their Oath of Professional, will be held on Monday and Tuesday, September 20 and 21, 2010 at 8:00 am and 1:00 pm respectively. This will be conductive before the Bard at the SMX Convention Center, SM Mall of Asia, Pasay City. All must come in their white gala uniform, nurse’s cap, white duty shoes, without earrings, hair not touching the collar and without corsage.

Oathtaking tickets for the National Capital Region will be available at the Philippine Nurses’ Association Office at 1663 F.T. Benitez Street, Malate, Manila, from September 1 – 20, on a “first come first serve” basis.

All regional oathtaking schedules will be posted in the Board of Nursing website: http://www.bonphilippines.org.

The resolution moves the test dates from December 12 and 13, 2010 to December 19 and 20, 2010 (Sunday and Monday), because latter dates fall on the Christmas vacation, which would allow the PRC to utilize public and private school in the administration of the exams.

Another factor would be that results for the July 2010 Philippine Nurses’ Licensure Examination will be released on the last week of August, 2010 and there will be very limited time for repeaters to file before the deadline September 08, 2010.

The NEW DEADLINE FOR FILING OF REPEATERS is now from September 8-28, 2010. Regular examinees will follow same schedule as set by PRC and is now on-going.

PREPARE THE FOLLOWING REQUIREMENTS:

1. Fully accomplished PRC Renewal form. You can download the form HERE. Print the form back-to-back, just make sure to cut it according to its original size. The PRC does not accept forms that are not within the size limit.
2. Two (2) pieces of your latest passport-sized picture in white background with name tag.
3. Two (2) photocopies of your PRC ID license (front and back).
4. Renewal fee of Php 450.00 (as of February 2011). *Please note that the renewal fee may increase depending on the months or years of lapse of validity of your license.
5. If you opt to have your license mailed, prepare a courier fee of Php 80.00 for Metro Manila Residents and varying prices, depending on your location.
6. It would also help if you are to bring your own ballpen and paste(To attach the picture in your renewal form).
7. For LOST PRC ID/License, please submit an affidavit of loss
8. For DESTROYED PRC ID/License, please submit the destroyed ID card.
9. Authorization letter for your representative, if you are not the one who would be processing.

STEP-BY-STEP PROCEDURE

STEP 1: Get a copy of the renewal form from PRC or DOWNLOAD it HERE.

STEP 2: Completely fill out the renewal form.

Place a CHECK MARK on what type of application you are to undertake for the professional ID.

Attach your picture by using a paste or glue.

Completely fill-up the portion high-lighted below.

Fill up the data need in the portion high-lighted below.

STEP 3: Present duly accomplished renewal form and other requirements to the Assessment Windows (WINDOW 16 for professions starting with letter “N”).

STEP 4: Pay prescribed fees (Php 450.00) at the Cashier.

STEP 5: Fill out additional details needed in the ID CLAIM SLIP portion of your form. (Amount, OR No., Date Paid)

STEP 6: Return to Window 16 at present form with receipt.

STEP 7: Secure ID CLAIM STUB. (IMPORTANT: Do not leave the counter without this claim stub.)

STEP 8: If you choose to have your ID mailed by courier, proceed outside by the photocopiers to WWWExpress. Fill out the form and present your ID CLAIM STUB along with your payment (Php 80.00 if within Metro Manila).

STEP 9: Obtain the carbon copy of your delivery receipt and ask when your license will be delivered to your home. (Usually, deliveries are done two (2) working days after the release of your ID.)
